What Marionette Lines Are
The name comes from the hinged jaw of a puppet, because these lines frame the chin the same way. They form when the fat pads at the corners of the mouth thin out and the skin loses elasticity, letting the corners of the mouth turn down and creating a crease from the corner toward the jawline. They're most visible at rest, which is what makes them frustrating. People tell you that you look unhappy when you feel fine.
How Filler Helps
Filler is placed at the corners of the mouth and along the line to restore the support that's been lost and lift the corners slightly. The effect is a softer crease and a mouth that rests more neutrally. This area moves constantly with talking and eating, so we use Radiesse, which is firm enough to hold its shape and stimulates collagen for a longer-lasting result. Many people also benefit from a small amount of Botox in the muscles that pull the mouth corners down, and we can do both in one visit.

What to Expect at Home
Dr. Bashian or Rachel Fey, PA will assess the lines at rest and while you talk, discuss whether Botox should be part of the plan, and apply numbing. Expect mild swelling or bruising for a few days. The lift is visible immediately and settles fully at two weeks. Radiesse lasts twelve to eighteen months for most people.
Who It's For
Adults with visible lines from the mouth corners toward the chin, or a downturned resting expression they'd like to correct. If the concern is more about the jowls than the lines themselves, filler along the jawline may be the better approach, and we'll discuss that. We do not treat anyone who is pregnant or breastfeeding.
Risks and Side Effects
Swelling, bruising and tenderness for a few days. Small nodules that usually resolve with massage. Because Radiesse cannot be dissolved, precise placement matters. Very rarely, filler can enter a blood vessel, which our providers are trained to recognize and manage. Individual results vary.
